We will be holding one last registration with football on May 23 from 5:30-7:30 pm at Perry Junior High.
We are accepting registration for all girls ages 5-13 as of July 31, 2012. At the time of registration you must hand in completed forms of the following:
- 2012 Medical form
- 2012 Player contract
- A physical dated no earlier than 1/12
- Original birth certificate for competition teams (see link) or a copy of the birth certificate for non-competition teams
All paperwork must be completed by July 15, 2012. NO EXCEPTIONS!
